From inquiry to care

Six stages keep the next step visible.

The details vary by client, but families should always understand what Harbor is reviewing, what information is still needed, and what comes next.

    Tell us what has changed at home.

    Share the routines that are becoming harder, who is providing support now, the schedule you are considering, and any immediate coverage questions.

    Confirm the request fits Harbor’s scope.

    We clarify whether Personal Care, Respite Care, or another next step best matches the need and explain the limits of Harbor’s non-medical services.

  3. 03

    Review needs and plan the support.

    Required intake and care-planning information is reviewed so the written service plan can reflect the person’s approved needs, routines, preferences, and safety considerations.

  4. 04

    Clarify coverage or payment.

    For Cardinal Care Virginia Medicaid, applicable eligibility and authorization details must be in place. For private pay, Harbor explains fees and the proposed service arrangement before care begins.

  5. 05

    Coordinate schedule and staffing readiness.

    Harbor works through the approved schedule, service-plan expectations, and caregiver assignment so everyone understands the arrangement before the first visit.

  6. 06

    Begin care and keep communication open.

    After services begin, Harbor maintains required supervision and communication and reviews changes that may affect the client’s needs, schedule, or written service plan.
