    Start with a conversation. Tell us about your loved one’s needs, the type of support you are considering, your preferred schedule, and any questions you may have.

    Complete a Personal Assessment

    We take time to understand the client’s routines, preferences, daily-living needs, and safety considerations. For personal-care services, Harbor’s supervising nurse completes an assessment.

  3. 03

    Create a Personalized Care Plan

    Harbor develops a written care plan describing the planned services, schedule, caregiver responsibilities, personal preferences, and important instructions for support at home.

    Prepare and Assign a Qualified Caregiver

    Harbor considers the client’s care needs, requested schedule, caregiver qualifications, relevant experience, communication style, and availability when assigning a caregiver.

    Begin Care With Ongoing Support

    Once services begin, Harbor remains available to coordinate services, answer questions, and review changes that may affect the client’s needs, schedule, or written care plan.
